The Mental Health Care Coordinator plays a critical role in improving access to behavioral health services by guiding patients through digital care programs and ensuring smooth communication between patients and providers.

Responsibilities

• Coordinate virtual mental health appointments
• Assist patients with behavioral healthcare enrollment
• Communicate with patients regarding care plans and services
• Support therapists, counselors, and clinical teams
• Maintain accurate patient records and documentation
• Help patients navigate digital healthcare platforms
• Track patient engagement and follow-up needs
• Provide compassionate patient support

Qualifications
