EverWell Medical Group is hiring a dedicated Virtual Care Nurse Coordinator to support our expanding telehealth programs throughout Texas. This remote nursing position is ideal for an organized Registered Nurse who enjoys combining clinical knowledge with technology-driven healthcare solutions.

The Virtual Care Nurse Coordinator will serve as a connection point between patients, providers, and digital healthcare teams. This role focuses on coordinating virtual appointments, reviewing care plans, supporting remote monitoring programs, and helping patients successfully navigate online healthcare services.

Responsibilities

• Coordinate virtual healthcare appointments and follow-up visits
• Support patients using telemedicine platforms
• Review patient care plans and clinical documentation
• Collaborate with physicians, nurses, and healthcare specialists
• Monitor remote patient care activities
• Assist patients with digital healthcare tools
• Document patient interactions in EHR systems
• Identify barriers affecting patient engagement and care access

Qualifications

• Active Registered Nurse (RN) license
• 2+ years of nursing experience preferred
• Experience with care coordination or case management
• Knowledge of telemedicine workflows
• Strong organizational and communication skills
• Comfort working with healthcare technology

Preferred Experience

• Virtual nursing experience
• Population health management
• Chronic care programs
• Remote patient monitoring

Technology Used

• Epic, Cerner, or similar EHR platforms

Schedule

Monday through Friday, flexible daytime schedule with occasional evening patient support rotations.
