CareConnect Health is expanding its virtual care team and is seeking experienced Remote Telehealth Registered Nurses (RN) to deliver high-quality healthcare services through secure telemedicine platforms. This work-from-home nursing opportunity supports patients managing chronic conditions, post-discharge recovery, preventive care programs, and ongoing healthcare needs.

The successful candidate will combine clinical nursing expertise with digital healthcare technology to perform virtual assessments, educate patients, coordinate care plans, communicate with providers, and improve patient outcomes.

Responsibilities

• Conduct virtual nursing assessments through telehealth platforms
• Provide patient education and health coaching
• Monitor chronic care management programs
• Review patient symptoms and escalate clinical concerns
• Coordinate care with physicians and healthcare teams
• Maintain accurate documentation within electronic health records (EHR)
• Protect patient information according to HIPAA standards

Qualifications

• Active Registered Nurse (RN) license
• Experience in clinical nursing, ambulatory care, or case management
• Strong communication and patient engagement skills
• Experience using EHR/EMR systems
• Ability to work independently in a remote environment
• Reliable high-speed internet connection

Preferred Experience

• Previous telehealth nursing experience
• Chronic disease management experience
• Care coordination background
