HealthBridge Virtual Network is seeking an experienced Remote Nurse Case Manager to join our growing virtual healthcare team. This work-from-home nursing opportunity focuses on helping patients navigate complex healthcare needs through telehealth services, care coordination, and digital health programs.

The Remote Nurse Case Manager will work with patients, physicians, specialists, and healthcare organizations to develop personalized care strategies, improve outcomes, and ensure patients receive the support they need throughout their healthcare journey.

Responsibilities

• Manage assigned patient caseload through virtual healthcare platforms
• Develop and coordinate individualized care plans
• Conduct remote patient assessments and follow-up visits
• Collaborate with physicians and multidisciplinary care teams
• Identify barriers to care and connect patients with resources
• Monitor patient progress and healthcare goals
• Document all clinical activities in electronic medical records
• Provide education on medications, treatments, and wellness strategies

Qualifications

• Active Registered Nurse (RN) license
• Case management or care coordination experience preferred
• Strong clinical assessment abilities
• Excellent communication and organizational skills
• Experience with EHR/EMR systems
• Ability to manage multiple patients remotely
