Chesapeake Advanced Medical Network is seeking an experienced Transplant Coordinator RN to support patients throughout the transplant care journey. This remote nursing position provides clinical coordination, patient education, appointment management, and communication support for transplant programs.

The Transplant Coordinator RN works closely with surgeons, physicians, nurses, patients, and caregivers to ensure patients receive organized and continuous specialty healthcare support.

Responsibilities

• Coordinate transplant patient care activities remotely
• Provide education and guidance to transplant patients
• Monitor patient follow-up requirements
• Coordinate appointments, testing, and clinical communication
• Review patient records and care plans
• Communicate with transplant teams and specialists
• Document patient interactions in electronic health records
• Support long-term transplant care management

Qualifications

• Active Registered Nurse (RN) license
• Critical care or specialty nursing experience preferred
• Strong clinical assessment skills
• Experience coordinating complex patient care
• Excellent patient education abilities
• Knowledge of electronic medical records

Schedule
