Great Lakes Population Health Network is seeking an experienced Population Health Analyst to transform healthcare data into meaningful insights that improve patient outcomes and strengthen virtual healthcare delivery. This fully remote position supports value-based care initiatives by identifying trends, evaluating clinical performance, monitoring quality metrics, and developing strategic recommendations for healthcare leaders.

The Population Health Analyst collaborates with physicians, care managers, clinical informaticists, quality improvement specialists, and executive leadership to improve preventive care, chronic disease management, and population health performance.

Key Responsibilities

• Analyze population health and telehealth performance metrics
• Develop executive dashboards and operational reports
• Identify healthcare trends using large clinical datasets
• Support value-based care initiatives
• Monitor quality measures including HEDIS and CMS metrics
• Collaborate with interdisciplinary healthcare teams
• Perform predictive and descriptive healthcare analytics
• Recommend workflow improvements based on data findings
• Ensure healthcare data integrity and accuracy
• Present analytical findings to leadership teams

Minimum Qualifications

• Bachelor's degree in Health Informatics, Public Health, Data Analytics, Statistics, or related field preferred
• Healthcare analytics experience preferred
• Knowledge of population health principles
• Experience with SQL and business intelligence reporting
• Advanced Microsoft Excel skills
• Strong analytical and communication abilities
