The Pharmacy Care Coordinator helps organize medication-related services, assists patients with pharmacy processes, and supports healthcare teams through accurate documentation and communication.

Responsibilities

• Coordinate pharmacy care management activities
• Assist patients with medication service questions
• Support medication adherence programs
• Communicate with pharmacies and healthcare providers
• Maintain accurate patient medication records
• Assist with prescription coordination workflows
• Document patient interactions
• Support virtual pharmacy operations

Qualifications
