North Star Healthcare Collaborative is seeking a Remote Hospital Discharge Planner to support patients transitioning from hospital care to home, rehabilitation, and community healthcare services. This remote healthcare position focuses on coordinating follow-up care, communicating with healthcare teams, and helping patients navigate post-hospital recovery.

The Discharge Planner works with nurses, physicians, social workers, and healthcare partners to create organized and patient-centered transition plans.

Responsibilities

• Coordinate hospital discharge planning activities remotely
• Review patient care needs and transition requirements
• Arrange follow-up appointments and healthcare services
• Communicate discharge information with patients and families
• Collaborate with clinical and administrative teams
• Support post-discharge care coordination
• Document transition activities in healthcare systems
• Identify barriers affecting successful recovery

Qualifications
