Sunshine Medical Network is seeking an experienced Remote Hospital Case Manager RN to support patients throughout their healthcare journey. This virtual nursing role focuses on care coordination, utilization management, discharge planning, and collaboration with multidisciplinary healthcare teams.

The Hospital Case Manager RN works remotely with physicians, hospitals, insurance partners, and patients to help ensure appropriate care delivery and successful health outcomes.

Responsibilities

• Manage assigned patient cases remotely
• Develop and coordinate individualized care plans
• Support hospital discharge and transition planning
• Communicate with patients and healthcare providers
• Review patient needs and available healthcare resources
• Coordinate follow-up appointments and services
• Document case management activities in electronic systems
• Participate in virtual care team meetings

Qualifications

• Active Registered Nurse (RN) license
• Hospital case management experience preferred
• Strong clinical assessment skills
• Experience with care coordination
• Excellent communication and organization skills
• Ability to manage multiple patient cases remotely

Schedule
