Evergreen Population Care is seeking a Population Health Nurse to join our remote healthcare team. This position focuses on improving health outcomes through virtual care management, preventive health programs, patient education, and data-driven healthcare support.

The Population Health Nurse works with patients, providers, and care teams to identify healthcare needs, develop outreach strategies, and provide personalized support for individuals managing chronic conditions and preventive care goals.

Responsibilities

• Manage virtual population health programs
• Conduct remote patient outreach and health assessments
• Educate patients about preventive care and wellness goals
• Identify high-risk patients needing additional support
• Coordinate care with providers and healthcare teams
• Review patient health information and care opportunities
• Document patient interactions within healthcare systems
• Support quality improvement initiatives

Qualifications

• Active Registered Nurse (RN) license
• Experience in population health, case management, or ambulatory care
• Strong analytical and communication skills
• Experience with electronic medical records
• Ability to engage patients remotely
• Comfort working with healthcare technology
