CareSphere Remote Health is looking for a dedicated Home Health Virtual Registered Nurse to support patients receiving healthcare services in their homes. This remote RN position focuses on virtual assessments, patient education, remote monitoring, and collaboration with home health teams.

The ideal candidate enjoys helping patients manage healthcare needs outside traditional clinical settings and understands the importance of proactive communication, care coordination, and personalized nursing support.

Responsibilities

• Perform virtual nursing assessments for home health patients
• Review remote patient monitoring information
• Provide education on treatment plans and wellness goals
• Coordinate care with home health clinicians and providers
• Monitor patient progress and identify care concerns
• Support patients using digital healthcare tools
• Document clinical information in electronic records
• Assist with preventive care and chronic condition management

Qualifications

• Active Registered Nurse (RN) license
• Home health, ambulatory, or community nursing experience preferred
• Strong patient communication skills
• Experience with care coordination
• Comfort using telehealth technology
• Reliable home internet connection
