Beacon Digital Healthcare is looking for an experienced Chronic Care Management Registered Nurse to join our virtual healthcare team. This remote nursing role supports patients living with long-term health conditions by providing education, monitoring, care coordination, and ongoing clinical support.

The Chronic Care Management RN partners with patients and healthcare providers to create better care experiences through proactive outreach, digital health tools, and personalized nursing interventions.

Responsibilities

• Manage assigned chronic care patient populations remotely
• Conduct scheduled virtual nursing assessments
• Provide education for chronic disease self-management
• Monitor patient progress toward healthcare goals
• Coordinate care between patients and providers
• Review health information and identify care opportunities
• Document patient interactions in electronic medical records
• Support patient engagement through telehealth platforms

Qualifications

• Active Registered Nurse (RN) license
• Experience in chronic care management preferred
• Strong clinical assessment skills
• Excellent communication and coaching abilities
• Experience with remote healthcare services
• Ability to work independently from home

Schedule

Flexible Monday through Friday schedule with optional evening patient outreach hours.
