VitalPath Healthcare Partners is seeking a dedicated Care Transition Nurse to support patients as they move between healthcare settings. This remote nursing position focuses on improving patient outcomes after hospital visits by providing virtual follow-up care, education, medication support, and coordination with healthcare providers.

The Care Transition Nurse will help reduce gaps in care by ensuring patients understand discharge instructions, follow treatment plans, and have access to the resources needed for successful recovery.

Responsibilities

• Conduct virtual post-discharge patient follow-up assessments
• Review discharge instructions and reinforce care plans
• Educate patients about medications and recovery expectations
• Coordinate appointments with primary care providers and specialists
• Identify potential barriers affecting patient recovery
• Communicate patient updates with healthcare teams
• Document interactions in electronic medical records
• Support readmission prevention initiatives

Qualifications

• Active Registered Nurse (RN) license
• Experience in hospital nursing or care transitions
• Strong patient education abilities
• Excellent communication skills
• Experience with electronic health records
• Ability to manage patient follow-up remotely

Schedule

Monday through Friday schedule with occasional weekend patient follow-up rotations.
