Pinnacle Health Solutions is seeking an experienced Cardiac Telehealth Nurse to join our virtual heart health team. This remote RN position supports patients managing cardiovascular conditions through digital healthcare solutions, remote monitoring programs, and coordinated virtual care services.

The Cardiac Telehealth Nurse will provide clinical guidance, monitor patient health information, educate patients on cardiac wellness, and collaborate with cardiologists and healthcare teams to improve long-term outcomes.

Responsibilities

• Monitor cardiac patients through remote healthcare platforms
• Review patient-reported symptoms and vital health information
• Provide virtual education related to heart health management
• Support care plans for hypertension, heart failure, and cardiovascular conditions
• Communicate changes in patient status to providers
• Coordinate follow-up appointments and healthcare resources
• Document clinical activities within electronic health records
• Assist patients with remote monitoring technology

Qualifications

• Active Registered Nurse (RN) license
• Cardiac nursing experience preferred
• Experience with chronic disease management
• Strong clinical assessment skills
• Comfort using digital healthcare platforms
• Excellent communication skills

Preferred Experience

• Cardiology department experience
• Heart failure programs
• Telemetry nursing
• Remote patient monitoring

Schedule

Options available for full-time weekday schedules, four 10-hour shifts, and rotating weekend coverage.
